I took about a foot of gutted black 550 paracord, and made a loop with a lanyard knot. I then used a couple of feet of light green 1mm cotton cord to make a cobra stitch(Solomon's bar/Portuguese sinnet) over the paracord. I used a couple of drops of super glue to secure the cotton cord. I made sure I had enough room to loop the fob thru the knife's lanyard hole.

Carry 50 feet of cord on your hat
After knitting the cord, I worked it around the webbing on the hat, starting at the back center, and finished off the last foot of cord, with the single start and end strands, using the snake knot.
I purchased the cord last year at an Army/Navy store and haven't seen it anywhere since. It's 1/16" polypropylene utility cord made by Atwood Rope Mfg. You'll have to google it and see if you can find a source for it, or use Type I paracord, or other 1/16" sized cord to get 50 feet of it around a boonie hat like I've done here. Of course this is a 7 3/4 sized hat to fit my large noggin, but I believe it'd still work on a smaller hat.

Paracord wrist lanyard made with the snake knot

USNSCC Wyoming Battalion and the 5th Annual Cody SOCOM Roundup
I gave a rough idea of the monkey fist with a cobra stitch/Solomon bar/Portuguese sinnet, and as you can see, they did a great job on the project. The guys made over a hundred of them and provided their creations to the Special Forces attendees of the 5th Annual SOCOM Roundup in Cody, Wyoming.
Thanks to Mark at the Supply Captain for assisting with the paracord for the project. And special 'Thanks!' to the cadets for their hard work and support for our troops.
